Here is a model template for collection from Baker Hostetler. Note the areas for you to add case specific and collector specific data (not all of those are in brackets).
You may also want to include the CEDS and any other certifications, and formal training in the software–or years of experience, number of collections with the software you or your collection team holds. If you have an SOP (standard operating procedure), you may want to reference that as well.
